Job summary

Lives Lived Well is seeking an AOD Counsellor / Case Manager for the Tokoko Integrated Recovery Pathways Program in Bundaberg. You will deliver counselling, case management and recovery-focused support to individuals across all recovery stages.

You will collaborate with a multidisciplinary team to strengthen treatment engagement, plan relapse prevention and coordinate connections to health, housing and community services. Experience in AOD services is highly regarded.

Qualifications

• Qualifications in Social Work, Counselling, Psychology, Community Services or related discipline or relevant experience.
• Demonstrated experience delivering counselling and/or case management services.
• Knowledge of Alcohol and Other Drug treatment, recovery and harm minimisation.
• Recovery-oriented, trauma-informed and person-centred practice understanding.
• Excellent interpersonal, communication and documentation skills.

Responsibilities

• Provide engagement, assessment, counselling and recovery-focused support.
• Develop, implement and review individual recovery and treatment plans.
• Deliver evidence-based counselling interventions.
• Support clients to identify and achieve recovery goals.
• Provide case management and coordinated support services.
• Assist clients to access health, housing, wellbeing and community supports.
• Support treatment readiness, recovery planning and relapse prevention.
• Maintain continuity of care through referrals and advocacy.
• Participate in multidisciplinary case discussions and service planning.
