Job summary

Central Adelaide Local Health Network (CALHN) is seeking a Placement Officer to support older people transitioning from hospital to residential aged care. You will coordinate applications, assist discharge planning and collaborate with patients, families and providers to find care options aligned with individual needs.

The role requires experience in acute, subacute or aged care settings, strong empathy, and competence in Microsoft Office.

Qualifications

• Excellent empathy and communication skills for patient-centred care.
