Job summary

We Care Staffing Solutions is seeking an experienced Care Partner to oversee delivery and coordination of in-home support services under My Aged Care programs in Narre Warren, VIC. You will assess clients, develop personalised plans, coordinate services and ensure safe, high-quality support aligned with client goals.

Ideal candidates have 3–5 years in the My Aged Care sector, knowledge of Aged Care Quality Standards, and current checks.

Qualifications

• 3–5 years in the My Aged Care sector with HCP and SAH packages.
• Strong knowledge of Aged Care Quality Standards and funding.
• Current Police Check and Full Work Rights; NDIS Check preferred.

Responsibilities

• Develop and grow the in-home support program with the Program Manager.
• Conduct intake assessments, identify client needs, risks, and goals.
• Case manage client’s care services, including in-home and community supports.
• Develop and review person-centred support plans.
• Coordinate and schedule services such as personal care, domestic support, community access or allied health services.
• Monitor and optimise care packages within budget and maintain documentation.
