Job summary

Anglicare NT in Australia is seeking a compassionate Care Finder Officer to support older Territorians in navigating aged care and community services. You will provide outreach, advocacy, and intensive support to help clients access the right services and improve wellbeing.

You will build strong relationships with clients, families, service providers, and community stakeholders, delivering person-centred assistance and ensuring culturally safe practices in line with organisational values.

Qualifications

• Diploma in Community Services or related field and/or significant experience in aged care, homelessness, or community services.
• Experience in case management, client engagement, and assertive outreach.
• Knowledge of the aged care sector, aged care reforms, and My Aged Care.
• Experience supporting vulnerable people with complex or changing needs.
• Excellent communication and relationship-building skills.
• Strong organisational and administrative skills.
• Confidence using technology, databases, and Microsoft Office applications.
• Commitment to culturally safe, inclusive, and person-centred practice

Responsibilities

• Provide outreach, advocacy, and intensive support to help clients navigate the aged care system, connect with appropriate services, and achieve improved wellbeing and independence.
• Build strong relationships with clients, families, service providers, and community stakeholders to ensure people receive support at the right time.
• Support older people who face challenges accessing aged care and community services in a compassionate, proactive manner.
