Job summary

Footprints Community Ltd in Queensland is seeking a dedicated Care Finder to provide specialist and intensive support to vulnerable senior Australians, helping them access aged care services and other supports with a person-centred, assertive outreach approach.

You will conduct holistic assessments, build relations with care providers, coordinate case conferences and ensure accurate reporting while upholding dignity, choice and self-determination.

Qualifications

• Tertiary qualification in Community Services or related field is required.
• Strong local networks with the care finder target population.
• Understanding of aged care supports and eligibility.
• Experience with diverse client groups including older people and those facing homelessness.
• Excellent interpersonal, verbal and written communication.
• Current Class C driver's license and vehicle access.
• NDIS Worker Screening Check or Yellow Card.
• Current First Aid and CPR certificate.

Responsibilities

• Undertake holistic assessments and client-led support planning.
• Conduct assertive outreach and build relationships with care providers.
• Provide warm referrals to My Aged Care and monitor engagement.
• Coordinate case conferences and interagency meetings.
• Maintain accurate client databases and complete reporting.
• Advocate for Footprints programs across sectors.
