Footprints Community Ltd in Australia is seeking a dedicated Care Finder to provide intensive support to vulnerable senior Australians, helping them access aged care services, including My Aged Care, with a person-centred outreach approach.

This is a full-time, max-term role classified under the SCHADS Award at Level 5. The salary range is $105,578 - $110,360 depending on skills and experience, plus superannuation and salary packaging benefits of up to $15,900 to increase your take-home pay.

Formación

• Tertiary qualification in Community Services or related field, or equivalent experience.
• Strong knowledge of aged care supports, pathways and eligibility.
• Experience with diverse client groups including older people and those with disabilities.
• Current Class C driver's licence and access to an insured vehicle.
• NDIS Worker Screening Check and First Aid/CPR certification.
• Excellent interpersonal, verbal and written communication.

Responsabilidades

• Undertake holistic assessments and client-led support planning.
• Conduct assertive outreach and build relationships with aged care and community providers.
• Provide warm referrals to My Aged Care and monitor ongoing client engagement.
• Coordinate and participate in case conferences, interagency meetings, and communities of practice.
• Maintain accurate client databases, high-standard case files, and complete required reporting.
• Advocate for service options and promote Footprints programs across networks.
