Job summary

Multicultural Care is seeking an experienced Clinical Aged Care Assessor to join our Care Navigations Team. In this community-based role, you'll conduct comprehensive aged care assessments to help older people access services while remaining independent, managing your own caseload within a supportive team.

You'll assess clients' clinical, functional and psychosocial needs, develop person-centred support plans, maintain records, and communicate with clients, families and health professionals.

Qualifications

• A tertiary qualification in nursing or allied health.
• Current professional registration or eligibility where applicable.
• Experience delivering services to older people, including people living with dementia.
• Ability to work independently and manage competing priorities.

Responsibilities

• Managing My Aged Care referrals and completing assessments within required timeframes.
• Assessing clients' clinical, functional and psychosocial needs, including risks and safeguarding concerns.
• Developing person-centred support plans and recommending appropriate services.
• Maintaining accurate records and meeting quality and productivity expectations.
• Communicating effectively with clients, families and health professionals.
