Job summary

Southern Cross Care (NSW & ACT) seeks a qualified Accredited Exercise Physiologist to deliver evidence-based clinical exercise interventions for older adults in residential care and in-home settings. You will plan tailored programs, monitor progress, and collaborate with nursing and allied health teams to ensure holistic care.

Ideal candidates hold ESSA accreditation, with experience in aged care, CHSP/AN-ACC knowledge, and a person-centered, collaborative approach within a not-for-profit

Qualifications

• Bachelor or postgraduate qualification in exercise physiology.
• Current ESSA accreditation as an Accredited Exercise Physiologist.
• Experience delivering clinical exercise interventions for older adults, chronic disease, rehabilitation or aged care settings (Desirable).
• Knowledge of AN-ACC, CHSP, and aged care funding programs (Desirable).
• Experience in home care, disability services, community health, or group-based exercise programs (Desirable).

Responsibilities

• Conduct evidence-based assessments and develop tailored exercise plans.
• Deliver interventions that support strength, balance, mobility, reablement, falls prevention, and chronic disease management.
• Monitor progress and adapt programs based on outcomes and changing needs.
• Empower clients and families through education, collaboration, and clear communication.
• Support safe, positive, and meaningful experiences for residents and clients.
• Collaborate with nursing, care, allied health, families and external providers for holistic care.
• Participate in reviews, case conferences, falls meetings and discharge planning.
• Build strong relationships to support service continuity.
• Guide Allied Health Assistants and contribute to team capability.
• Maintain accurate, timely documentation per standards.
• Support quality improvement, audits, incident management, and risk mitigation.
• Contribute to safe work practices including infection control and WHS.
