Southern Cross Care (NSW & ACT) seeks a qualified Accredited Exercise Physiologist to deliver evidence-based clinical exercise interventions for older adults in residential care and in-home settings. You will plan tailored programs, monitor progress, and collaborate with nursing and allied health teams to ensure holistic care.

Ideal candidates hold ESSA accreditation, with experience in aged care, CHSP/AN-ACC knowledge, and a person-centered, collaborative approach within a not-for-profit

Qualifikationen

• Bachelor or postgraduate qualification in exercise physiology.
• Current ESSA accreditation as an Accredited Exercise Physiologist.
• Experience delivering clinical exercise interventions for older adults, chronic disease, rehabilitation or aged care settings (Desirable).
• Knowledge of AN-ACC, CHSP, and aged care funding programs (Desirable).
• Experience in home care, disability services, community health, or group-based exercise programs (Desirable).

• Maintain accurate, timely documentation per standards.
• Support quality improvement, audits, incident management, and risk mitigation.
• Contribute to safe work practices including infection control and WHS.

What You'll Do

Client & Service Delivery

• Work closely with nursing, care, allied health, families, and external providers to ensure coordinated, holistic care.
• Participate in reviews, case conferences, falls meetings and discharge planning activities.
• Build strong relationships with internal and external stakeholders to support service continuity.
• Guide Allied Health Assistants and contribute to team capability.

Quality, Safety & Continuous Improvement

• Maintain accurate, timely documentation in alignment with organisational and legal standards.
• Support quality improvement, audits, incident management, and risk mitigation activities.
• Contribute to safe work practices, including infection control, manual handling, and WHS requirements.
• Use outcome data, observations, and feedback to refine practice and improve service delivery.

Education and Experience

• Relevant bachelor or postgraduate qualification in exercise physiology, clinical exercise physiology, or equivalent pathway recognised for ESSA Accredited Exercise Physiologist accreditation.
• Current ESSA accreditation as an Accredited Exercise Physiologist.
• Experience delivering clinical exercise interventions for older adults, chronic disease, rehabilitation, or aged care settings (Desirable).
• Knowledge of AN-ACC, CHSP, and aged care funding programs (Desirable).
• Experience in home care, disability services, community health, or group-based exercise programs (Desirable).

Skills and Capability

• Skilled in assessment, clinical reasoning, exercise prescription, program delivery, monitoring, and progression.
• Knowledge of age-related conditions, chronic disease, falls prevention, strength and balance training, mobility support, and reablement.
• Strong documentation, time-management, and organisational abilities.

• Confidence in group facilitation, education, behaviour-change coaching, and providing specialist exercise advice.
• Familiarity with outcome measurement and using data to improve practice (Desirable).
