Job summary
At Home Together in Canberra is seeking an Aged Care Coordinator / Care Partner to join our team. You will manage a portfolio of home care clients, lead care planning, onboarding and reviews, working with families and healthcare professionals.
Preference for Enrolled Nurses or Registered Nurses with experience in community aged care; you will coordinate services, monitor wellbeing, and ensure compliance with care standards. A driver’s licence and willingness to travel locally are required.
Qualifications
- Qualified EN or RN with current AHPRA registration.
- Experience in community nursing, aged care coordination or home care.
- Knowledge of My Aged Care, care planning and client assessments.
- Portfolio management of aged care clients with multidisciplinary coordination.
Responsibilities
- Manage an allocated portfolio of aged care clients under the Support at Home program.
- Conduct initial assessments, onboarding and regular home visits.
- Develop and review individualised care plans and risk assessments.
- Coordinate referrals to nursing and allied health professionals.
- Monitor client wellbeing and service utilisation; maintain records.
- Engage with hospitals, GP clinics, and community organisations.
