Job summary

One Dream Community is seeking a Registered Nurse or Enrolled Nurse to join our Sydney team in Burwood as an Aged Care RN Coordinator. The role focuses on coordinating care and services for older clients receiving support through the Support at Home program, including assessments and care plan development.

You will work with clients, families and health professionals to ensure timely services, clinical governance and culturally responsive care in home settings across NSW.

Qualifications

• Current nursing registration with AHPRA.
• Experience in community aged care/home care.
• Understanding of person-centred care in home settings.
• Strong assessment, care planning and coordination skills.
• Proficient in clinical documentation and record keeping.
• Excellent communication and organisational abilities.

Responsibilities

• Conduct home visits and comprehensive client assessments.
• Develop, implement and review individualised care plans.
• Coordinate Support at Home services per assessed needs and budgets.
• Monitor client wellbeing and changing care needs.
• Collaborate with GPs, allied health, hospitals and providers.
