DANS In Home Care in Dubbo, NSW is seeking an experienced Care Partner to join our team and coordinate supports for older Australians living at home. You will assess clients, develop personalised care plans, monitor budgets, and work with families and healthcare providers to ensure services align with goals and quality standards.

The role offers a full-time, on-site position in a supportive, regional organisation with strong community connections and opportunities for professional development.

Qualifications

• Experience in aged care, community care, disability services, nursing, allied health, or care coordination.
• Strong communication and relationship-building skills.
• Ability to manage competing priorities and work autonomously.
• Sound computer skills and confidence using client management systems.

Responsabilités

• Manage a portfolio of aged care clients receiving government-funded supports.
• Conduct client assessments, reviews, and goal planning.
• Develop and coordinate personalised support plans.
• Monitor client budgets and service utilisation.
• Build strong relationships with clients, families, advocates, and service providers.
• Ensure services are delivered in line with client goals and quality standards.
• Maintain accurate client records, documentation, and case notes.
• Support clients to navigate aged care services and funding arrangements.
• Contribute to continuous improvement and positive client outcomes.
