Job summary

At Home Together in Canberra is seeking a compassionate Aged Care Coordinator / Care Partner to oversee care journeys for an allocated portfolio of clients. ENs or RNs with community aged care experience are preferred to lead assessments, care planning and regular reviews.

You will collaborate with clients, families, healthcare professionals and support workers to deliver safe, person-centred in-home services under the Support at Home program, while maintaining records and ensuring compliance.

Qualifications

• Qualified EN or RN with current AHPRA registration.
• Experience in community aged care, care coordination or home care.
• Experience with the Support at Home program or Home Care Packages.
• Knowledge of care planning, client assessments and service delivery.
• Ability to manage a portfolio of clients and coordinate multidisciplinary services.

Responsibilities

• Manage a portfolio of aged care clients, from onboarding to ongoing care coordination.
• Develop, implement and review individual care plans and risk assessments.
• Coordinate home care services and maintain accurate records.
• Work with families, healthcare professionals and providers to ensure safe, person-centred care.
• Maintain compliance with Aged Care Quality Standards and organisational policies.
• Engage with community and referral sources to grow services.
