Job summary

Respect in Warrnambool is seeking a Coordinator Community Programs to lead delivery of community programs and HACC-PYP services for eligible participants, promoting independence and social connection.

Reporting to the Manager Wellness & Reablement Services, you will coordinate referrals, onboarding, assessments, and care plans, while working with families, volunteers and community partners to achieve positive outcomes.

Qualifications

• Experience in aged care, community services, disability, health or a related sector.
• Experience coordinating services, programs, activities or participant supports.
• Strong understanding of person-centred care, wellness and reablement approaches.
• Strong relationship-building skills with participants, families, carers and stakeholders.
• Excellent organisation and time management skills, with the ability to manage competing priorities.
• Confident written, verbal and interpersonal communication skills.
• Ability to work independently and collaboratively as part of a team.
• Strong attention to detail and experience maintaining accurate records, documentation and reports.
• Confidence using client management systems and Microsoft Office.
• Current First Aid certification, National Police Check (or willingness to obtain) and a current Driver Licence.

Responsibilities

• Coordinate community programs and services that promote independence, wellbeing, social connection and meaningful participation.
• Manage the participant journey, including referrals, onboarding, assessments, service commencement and ongoing reviews.
• Develop and maintain individual care plans and goals, ensuring services reflect changing needs, preferences and desired outcomes.
• Connect participants with programs and community opportunities that match their interests, goals and support needs.
• Build positive relationships with participants, families, carers, volunteers and community partners, responding to enquiries and feedback with care and professionalism.
• Coordinate program operations, including schedules, resources, communications and service providers, to support smooth and effective delivery.
• Monitor service quality and participant outcomes, maintaining accurate records, identifying risks and escalating issues when required.
• Champion wellness and reablement, contributing to continuous improvement, quality initiatives, community engagement and compliance requirements.
