Job summary

Respect Group in Warrnambool is seeking a Coordinator Community Programs to lead the coordination and delivery of community programs and HACC-PYP services, creating meaningful opportunities for participants to achieve goals and stay active.

Reporting to the Manager Wellness & Reablement Services, you will coordinate participant services, facilitate access to activities and work with families, volunteers and community partners to deliver high-quality care that reflects individual needs and

Qualifications

• Experience in aged care, community services, disability, health or a related sector.
• Experience coordinating services, programs, activities or participant supports.
• Strong understanding of person-centred care, wellness and reablement approaches.
• Strong relationship-building with participants, families and stakeholders.
• Excellent organisation and time management, ability to manage competing priorities.
• Confident written, verbal and interpersonal communication.
• Ability to work independently and in a team.
• Attention to detail and ability to maintain accurate records.

Responsibilities

• Coordinate community programs and services promoting independence, wellbeing, social connection and meaningful participation.
• Manage the participant journey including referrals, onboarding and reviews.
• Develop and maintain individual care plans and goals reflecting changing needs.
• Connect participants with programs and opportunities matching interests and support needs.
• Build positive relationships with participants, families, carers, volunteers and community partners.
• Coordinate program operations, schedules, resources and service providers.
• Monitor service quality and participant outcomes, maintain records and escalate risks as needed.
• Champion wellness and reablement and contribute to continuous quality improvement.
