Community Accessability in Wangaratta seeks a dedicated HACC PYP Coordinator to lead the Home and Community Care Younger People program within our Aged Care division. You will oversee operations, coordinate care delivery, and ensure compliance with regulatory standards.

Qualifications

Responsibilities

• Coordinate the HACC PYP program for Younger People in line with legislation and policies.

We are seeking a dedicated and experienced HACC PYP Coordinator to join our team at Community Accessibility in Wangaratta. As the HACC PYP Coordinator, you will play a pivotal role in our Aged Care division, overseeing and coordinating key operations to ensure the delivery of high-quality services to our clients. Your responsibilities will include:

• Managing and coordinating the Home and Community Care (HACC) Program for Younger People (PYP) in accordance with legislative requirements and organisational policies


• Assessing the needs of clients and developing individualised care plans to support their independence and wellbeing


• Overseeing a team of support staff, providing guidance, training, and supervision to ensure exceptional service delivery


• Liaising with external service providers, stakeholders, and community organisations to facilitate access to additional support services for clients


• Collaborating with internal departments to streamline processes and improve overall operational efficiency


• Maintaining accurate records, reports, and documentation in compliance with regulatory standards


• Participating in continuous quality improvement initiatives to enhance the effectiveness of the HACC PYP program


• Contributing to the development of policies, procedures, and strategies to meet the evolving needs of our client base
