Job summary

Echo Community Services invites applications for two Case Manager/Coordinator roles. Based in Perth, you will coordinate in-home and community aged care services, manage care plans, and advocate for clients under CHSP and SAH funding streams.

Qualifications

• Certificate III or IV in Aged Care or equivalent with case management experience.
• Strong organisational skills and ability to work under pressure.
• Proficient with Microsoft Office and CRM systems including ECASE.

Responsibilities

• Case manage client care services including in-home and community activities.
• Oversee care planning, client clinical care, and case management.
• Ensure services meet client needs under CHSP/SAH funding streams.
• Maintain confidential records and ensure quality and compliance.
