Job summary

SA Health's Eyre and Far North Local Health Network is seeking a Home Care Coordinator for Country Health Connect in Streaky Bay on a part-time basis, 60 hours per fortnight, until 31 December 2026. You will coordinate Commonwealth-funded aged care programs, assess clients, and develop person-centred care plans to help older people stay independent.

Experience in case management and knowledge of CHSP/Support at Home are essential, with strong multi-disciplinary collaboration and regional service

Qualifications

• Demonstrated experience in case management and coordinating care for people with complex or multiple needs.

Responsibilities

• Conduct holistic assessments and develop, implement and review person-centred care plans.
• Coordinate referrals and services across health, community and government providers.
• Advocate for clients while promoting independence, choice and self-advocacy.
• Contribute to service improvement through data collection, evaluation and quality initiatives.
