Austa Care, under GCAA Health, seeks a Chinese-speaking Case Manager to lead end-to-end home aged care services for SAH clients in Sydney or Melbourne. You will oversee intake, assessments, care planning, service authorisation, and risk management while guiding junior staff and support workers.
Applicants need 2–3 years of case management experience in Australian home aged care, a relevant Diploma or higher, and fluency in Mandarin.
As a full-time Case Manager based in Sydney or Melbourne, you will report to the CEO and lead the end-to-end journey for Support At Home (SAH) aged care clients. You will oversee participant intake, assessment, care planning, service authorisation, and risk management while guiding Assistant Case Managers and support workers to ensure high-quality, compliant care.
Manage referrals, conduct comprehensive physical, cognitive, psychosocial, and environmental assessments, sign service agreements, and determine eligibility for SAH or CHSP services
Create goal-oriented, personalised care plans within 5 business days of assessment, covering required services, frequency, budget allocation, and risk management strategies
Authorise third-party providers, manage out-of-scope requests, review support notes and participant feedback, and adjust care plans alongside Assistant Case Managers as client needs evolve
Track changes in client health, manage clinical risks (e.g., falls, medication errors), implement corrective actions for incident reports, and document risk events in line with standards
Monitor client spending to maintain target budget variances, reallocate funds per guidelines, coordinate with finance on invoicing, and provide clear budget updates to clients
Maintain complete client records, participate in internal audits, and stay informed on SAH reforms and Aged Care Quality Standards
Lead care workers, chair participant case conferences, and collaborate across clinical, finance, and quality departments
Minimum 2-3 years of Case Management experience in Australian home aged care with essential SAH experience
Diploma level qualification required (Bachelor preferred) in Nursing, Allied Health, Social Work, Aged Care, or Community Services
Deep understanding of Aged Care Quality Standards and Support At Home
Proficiency in MS Office and aged care software (e.g., Visual Care)
Strong background in budget management and financial reporting
Fluency in Mandarin is required; Cantonese is highly preferred
Valid Aged Care Worker Screening Clearance
Valid driver's licence and access to a reliable vehicle for community visits
