Qualifications

• Understanding of the new aged care act and support at home program.
• Full drivers licence required.
• Certifications in aged care, disability or community services relevant to case management.
• Experience in case management and coordination within a care setting.

Responsibilities

• Manage a caseload of clients with assessments, care planning and regular reviews.
• Coordinate and monitor in-home services with clients, families and providers.
• Ensure compliance with guidelines and maintain accurate documentation.
