Job summary

Good Shepherd Lodge Ltd. in Mackay, Queensland, seeks a Clinical Care Partner to provide clinical governance and leadership across Home Community Care on a part-time basis. You will work with care coordinators, allied health professionals, and staff to deliver safe, person-centred services in clients’ homes.

The role focuses on governance, clinical quality, and workforce capability, while supporting both clients and frontline teams to achieve optimal health outcomes.

Qualifications

• Current registration as a Registered Nurse with AHPRA.
• Experience in community aged care or complex care management is preferred.
• Strong clinical assessment, care planning and risk management abilities.
• Knowledge of aged care legislation, standards and funding models is advantageous.

Responsibilities

• Provide clinical governance and support to community care staff.
• Conduct assessments, care planning, and reviews aligned with best practice.
• Monitor client wellbeing and identify risks with timely interventions.
• Support wound care, medication management, continence, falls prevention and chronic disease management.
• Ensure documentation and reporting meet funding program requirements.
• Liaise with GPs, hospitals and health providers to support continuity of care.
• Mentor and coach frontline staff and promote continuous improvement.
