Job summary

Justice Health NSW in Bathurst seeks an Aboriginal Health Practitioner/Aboriginal Health Worker to join the Primary Care team. You'll deliver flexible, holistic, culturally safe health care to Aboriginal and/or Torres Strait Islander people, focusing on prevention and management of chronic disease and continuity of care within custody to support transition back to community.

Cert IV or III qualifications required with AHPRA registration for Practitioner; travel in NSW may be needed; role

Qualifications

• Holds or aims to hold relevant Aboriginal health care qualifications and be registered with AHPRA.
• Aboriginal Health Practitioner/Worker pathway with clear certification requirements.

Responsibilities

• Provide holistic, culturally safe health care to Aboriginal and/or Torres Strait Islander people.
• Collaborate with nursing, Integrated Care Service, and other health and community groups.
• Focus on prevention and management of chronic disease and support custody-to-community transitions.
