Job summary

NSW Justice Health & Forensic Mental Health Network is seeking an Aboriginal Health Worker/Practitioner, Primary Care based in Broken Hill. The role focuses on culturally safe care for Aboriginal and/or Torres Strait Islander clients and involves collaboration with a multidisciplinary team in a correctional health setting.

Eligible candidates hold or aspire to appropriate Certificates and AHPRA registration, with travel as required.

Qualifications

• Aboriginal Health Practitioner with Certificate IV in Aboriginal Primary Health Care (Practice) and registered with AHPRA.
• Aboriginal Health Worker with minimum Certificate III in Aboriginal Primary Health Care; transition to Cert IV possible with support.
• Eligible to drive in NSW and willing to travel as required.

Responsibilities

• Provide flexible, holistic, culturally sensitive health care to Aboriginal and/or Torres Strait Islander people.
• Focus on prevention and management of chronic disease for Aboriginal clients.
• Collaborate with nursing, Integrated Care Service, health professionals, Corrective Services NSW and other groups to ensure seamless care.
