Justice Health and Forensic Mental Health Network seeks an Aboriginal Health Worker/Practitioner for Primary Care at Bathurst. This temporary full-time role supports Aboriginal and Torres Strait Islander patients within a multidisciplinary team, focusing on prevention and chronic disease management.

Qualifications

• Qualified Aboriginal Health Professional with registration in Australia.
• Certificate IV/Certificate III in Aboriginal Primary Health Care.
• Eligible to drive in NSW and able to travel as required.

Responsabilités

• Provide flexible, holistic, culturally sensitive primary health care to Aboriginal and/or Torres Strait Islander people.
• Collaborate with nursing, Integrated Care Service, health professionals and community groups to ensure timely care.
