Job summary

Justice Health NSW is seeking an Aboriginal Health Worker/Practitioner, Primary Care - Broken Hill, to join a multidisciplinary health centre team. You will deliver patient-centred care to Aboriginal and/or Torres Strait Islander people, with emphasis on prevention and chronic disease management in a correctional health setting.

The role requires eligibility for Certificate IV or III in Aboriginal Primary Health Care, AHPRA registration for the Practitioner track, and willingness to travel

Qualifications

• Holds or is eligible for Certificate IV/III in Aboriginal Primary Health Care as applicable.
• Registered with AHPRA if applying as Aboriginal Health Practitioner.

Responsibilities

• Provide flexible, holistic, culturally sensitive health care to Aboriginal and/or Torres Strait Islander people.
• Collaborate with nursing staff, Integrated Care Service and external partners to ensure timely, seamless care.
• Support chronic disease prevention and management, helping clients re-enter community from custody when applicable.
