Job summary

Derbarl Yerrigan Health Service Aboriginal Corporation seeks an Aboriginal Health Practitioner or Nurse to join its Specialist Clinic team in Perth, coordinating services and supporting patients throughout their healthcare journey.

The role involves screening, patient safety, and arranging specialist/allied health appointments within a culturally safe framework across multiple DYHS clinics.

Qualifications

• Certificate IV or Diploma in Aboriginal Health or Enrolled Nurse.
• Current Registration with the Australian Health Practitioner Regulation Agency as an Aboriginal Health Practitioner or Enrolled Nurse.

Responsibilities

• Coordinate specialist clinic services and provide high-quality support to patients.
• Conduct client screening and check vital signs.
• Coordinate allied health and specialist appointments and liaise with external government and NGOs.
• Make appointments and arrange transport for clients as needed.
• Develop and review management plans with other health professionals.
