Job summary

Maari Ma Health in Broken Hill (Far West NSW) is seeking a Connecting Care Worker to support Aboriginal and Torres Strait Islander people to access culturally safe health, aged care, disability, housing and community services. You will work with clients, families and health professionals to provide advocacy, care coordination, referral support and health system navigation.

The role focuses on older Aboriginal people over 55 who are frail or vulnerable, with outreach to Menindee and Ivanhoe.

Qualifications

• Supports Aboriginal and Torres Strait Islander people to access culturally safe health, aged care, disability, housing, social and community services.
• Provides advocacy, care coordination, referral support, health system navigation and practical assistance.
• Promotes holistic, culturally responsive care aligned with community values.
• Priority client group: Aboriginal people aged over 55 years who are frail or vulnerable.
• Outreach to Menindee and Ivanhoe from Broken Hill.
