Job summary

Qualifications

• NSW Working with Children Check required (cleared or supported to obtain)
• National Police Check required
• Immunisation policy compliance is mandatory

Responsibilities

• Advocacy, care coordination, referral support and health system navigation for Aboriginal and Torres Strait Islander clients.
• Provide practical assistance to access culturally safe health, aged care, disability, housing, social and community services.
• Support clients to improve health, wellbeing and independence, including outreach as needed.
