The Aboriginal Mental Health Care Navigator within Sydney Local Health District will ensure culturally safe, trauma-informed access to Aboriginal Community Mental Health services for Aboriginal and Torres Strait Islander consumers. This role builds links across community mental health, NGOs, primary care, and social support services to streamline care transitions.

Qualifications

• Aboriginal and Torres Strait Islander knowledge with community links.
• Bachelor Health Science (Mental Health) or AHPRA eligibility/registration.
• Experience with case management and care planning.

Responsibilities

• Coordinate access to Aboriginal Community Mental Health services.
• Foster collaboration across community, NGO, primary care and services.
• Support transitions in care for Aboriginal consumers.
