Key Responsibilities:

Provide professional nursing care to patients in their homes, including post-operative care, wound care, medication administration, and chronic disease support.

Conduct comprehensive assessments and develop individualized care plans in collaboration with the broader healthcare team.

Educate patients and families on treatment protocols, self-care, and health maintenance.

Monitor patient progress and communicate changes to physicians or care coordinators promptly.

Maintain accurate clinical documentation and ensure compliance with local health regulations and organizational policies.

Requirements
