Key Responsibilities

• Provide expert consultation to patients and families on pain management, symptom control, and psychosocial support during serious and end-of-life illness
• Conduct comprehensive assessments addressing physical, emotional, social, and spiritual needs
• Develop and implement individualized palliative care plans in collaboration with multidisciplinary teams
• Coordinate and monitor care across hospital, hospice, long-term care, and home care settings
• Guide patients and families through complex medical decisions, including goals of care and end-of-life planning
• Educate and train physicians, nurses, and allied healthcare staff on palliative care principles and best practices
