Job summary

YOACAP, a program of Urban Affairs Coalition, seeks a dedicated Case Manager to staff the hotline and field inquiries from barbers, community members and youth. You will help clients access services and navigate systems, tailoring support to each person's needs and barriers.

You may assist with online applications, gather paperwork, and arrange warm referrals to partner organizations, following up to ensure services are received. This is a full-time role with a comprehensive benefits package.

Qualifications

• Experience in social services or case management preferred.
• Strong communication and interpersonal skills.
• Ability to navigate resources and assist clients with paperwork.

Responsibilities

• Staff the hotline and field inquiries from barbers, community members and youth.
• Help clients understand available programs and services and assist with accessing them.
• Complete online applications, call agencies, and gather required paperwork as needed.
• Coordinate with partners and follow up to ensure services are received and barriers are addressed.
