Placements24 is seeking a skilled Social Support Specialist to join our Sandton team in a hybrid role that blends on-site client engagement with remote work. You will provide comprehensive social support to individuals and families, helping them navigate challenges and access resources while fostering resilience within the local community.

The role requires proactive identification of needs, coordinating interventions, and contributing to the social well-being of residents in Gauteng.

Qualifications

Degree in Social Work, Psychology, Community Development, or related field.
Excellent communication, active listening and problem-solving skills.
Proven ability to manage hybrid workload (on-site and remote).

Responsibilities

Conduct comprehensive social needs assessments and develop tailored support plans.
Provide counseling, emotional support, and practical assistance to individuals and families.
Facilitate access to community resources including healthcare, housing, financial aid, and employment services.
Maintain accurate, confidential client records and document interactions.
Collaborate with partner agencies and service providers for holistic support.
Perform on-site visits and remote case management and follow-up.
