About the Role

Key Responsibilities

• Conduct thorough assessments of client needs, including risk assessments and care plan development.

• Monitor client progress, regularly review care plans, and adjust interventions as needed.
• Maintain detailed and confidential case notes and reports in compliance with organizational policies.
• Organize and facilitate community-based programs and support groups.

Requirements
