Qualifications

• Registered Pharmacist with SAPC.
• 3–5 years of experience in community or clinical pharmacy.
• Experience in MTM or clinical pharmacy services.
• Proficiency with EHR systems and telehealth platforms.
• Strong communication and counseling skills.

Responsibilities

• Provide remote medication management services, including reconciliation and therapy optimization.
• Conduct virtual patient counseling sessions on medications, side effects, and lifestyle management.
• Collaborate with physicians and healthcare teams for effective drug therapy.
• Review and interpret electronic prescriptions and patient health records.
• Identify and resolve drug-related problems like interactions and allergies.
• Utilize telehealth platforms and EHRs to deliver patient care.
• Participate in remote interdisciplinary care discussions.
• Develop educational materials for patients and providers.
• Ensure compliance with pharmacy laws and standards.
• Contribute to continuous improvement of remote pharmacy services.
