Qualifications

• Relevant qualification in Social Work, Occupational Therapy, Disability Studies, or a related field.
• Minimum of 3 years' experience in disability services or support coordination.
• In-depth knowledge of the disability sector, available resources, and relevant legislation.
• Excellent organizational and planning skills, with meticulous attention to detail.
• Strong interpersonal and communication skills, with proven ability to build rapport remotely.
• Proficiency with digital tools for communication and case management.

Responsibilities

• Coordinate support services for individuals with disabilities, aligning with their goals and preferences.
• Develop, implement, and review personalized support plans in collaboration with clients and their families.
• Liaise with service providers to ensure effective service delivery.
• Provide advocacy and support to clients navigating the disability support system.
• Maintain accurate client records and case notes through the online portal.
• Conduct regular remote check-ins and assessments to monitor client well-being and satisfaction.
