Placements24 is seeking a dedicated Disability Support Practitioner to provide person-centered care to individuals with disabilities in Stellenbosch, Western Cape. You will help clients achieve personal goals, promote independence, and collaborate with families and professionals to ensure holistic care.

Qualifications

• Relevant qualification in disability care, social work, or a related health field.
• Demonstrated experience supporting individuals with various disabilities.
• Strong understanding of disability rights and person-centered approaches.

Responsibilities

• Develop and implement individualized support plans in consultation with clients and their support networks.
• Assist clients with daily living activities, personal care, and mobility as required.
• Facilitate participation in community activities, social events, and educational pursuits.
• Provide emotional support and promote self-advocacy among clients.
• Monitor client progress, document observations, and report any changes or concerns to the case manager.
• Administer medication as directed and follow all health and safety procedures.
• Collaborate with families, therapists, and other service providers to ensure coordinated care.
