Qualifications
- Bachelor's degree in Social Work, Disability Studies, Occupational Therapy, or related field.
- 3-4 years of disability support or case management experience.
- Familiarity with disability legislation and NDIS-like principles in SA.
Responsibilities
- Conduct remote needs assessments for individuals with disabilities.
- Develop personalized support plans with clients and families.
- Coordinate provision of support services and assistive technologies.
- Liaise with service providers and community organizations.
- Monitor and review support plans; adjust as needed.
- Provide guidance and advocacy to clients and families.
