About the Role
Our client is seeking a compassionate and skilled Disability Support Practitioner to join their dedicated team, serving the Pretoria area. This role is instrumental in empowering individuals with disabilities to live fulfilling and independent lives. You will provide personalized support, promoting autonomy, dignity, and inclusion within the community. Working remotely, you will coordinate care, provide emotional support, and liaise with families and allied health professionals to ensure holistic well-being for clients throughout Gauteng .
Key Responsibilities
- Develop and implement individualized support plans tailored to the specific needs and goals of each client.
- Provide direct support, including personal care, assistance with daily living activities, and community participation.
- Act as a key liaison between clients, families, healthcare providers, and support services, ensuring effective communication and coordination of care.
- Facilitate client engagement in recreational, educational, and vocational activities, promoting independence and social inclusion.
- Maintain accurate and up-to-date client records, including progress notes, incident reports, and service delivery documentation.
- Respond to client needs promptly and professionally, ensuring their safety, comfort, and well-being at all times.
Requirements
- Relevant qualification in Disability Studies, Community Services, or a related field.
- Proven experience in supporting individuals with various disabilities.
- Strong understanding of disability rights, person-centered approaches, and inclusive practices.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work autonomously and manage time effectively in a remote setting.
- Proficiency in using digital communication tools and electronic record-keeping systems.
