Placements24 is seeking a Rehabilitation Coordinator for our Mahikeng, North West facility. You will oversee the development and coordination of substance abuse rehabilitation programs, working with therapists, counselors, and medical staff to ensure comprehensive client care.

The role requires a tertiary qualification in psychology, social work, or counseling, at least 4 years in related fields, and proficiency with EHR/software.

Competenze

• A relevant tertiary qualification in Psychology, Social Work, Counseling, or a related field.
• Minimum of 4 years of experience in substance abuse counseling, rehabilitation, or related field.
• In-depth knowledge of addiction recovery models, relapse prevention techniques.
• Strong case management, assessment, and therapeutic intervention skills.
• Excellent interpersonal and communication abilities.
• Proficiency with electronic health records (EHR) or similar software.

Mansioni

• Develop, implement, and manage individualized rehabilitation plans for clients with substance abuse disorders.
• Coordinate and oversee the delivery of therapeutic services, including individual and group counseling.
• Liaise with clients, families, and external agencies to ensure continuity of care.
• Monitor client progress and document milestones and challenges.
• Facilitate psychoeducational groups and life skills training sessions.
• Conduct initial assessments and ongoing evaluations of client needs and treatment effectiveness.
• Ensure adherence to program protocols, ethical guidelines, and regulatory requirements.
• Provide support and guidance to clients in developing relapse prevention strategies.
• Contribute to the development and refinement of rehabilitation program content and methodologies.
