Job summary

Placements24 is seeking a dedicated Case Manager in Pretoria to support individuals in a Substance Abuse Services program. You will guide recovery, provide comprehensive case management, and connect clients with treatment resources in Gauteng.

The role involves assessing needs, coordinating services, maintaining records, and facilitating relapse prevention groups to empower clients in their recovery journey.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, or related field.
• Experience in case management, especially with substance abuse clients.
• Knowledge of addiction, recovery processes, and community resources in South Africa.
• Strong organizational, problem-solving, and communication skills.
• Empathy, patience, and ability to build rapport with diverse clients.
• Familiarity with electronic health record systems is an advantage.

Responsibilities

• Conduct thorough client assessments to determine eligibility and develop treatment plans.
• Provide ongoing case management, including crisis intervention, advocacy, and support.
• Coordinate access to services such as counseling, medical care, housing, and employment assistance.
• Monitor client progress and adjust plans as needed.
• Maintain confidential client records in line with policies and regulations.
• Facilitate psychoeducational groups and relapse prevention support.
