Placements24 is seeking a Rehabilitation Coordinator for a substance abuse program in Mahikeng, North West. You will oversee development and coordination of rehabilitation plans, ensuring comprehensive client care with a multidisciplinary team, while documenting progress and outcomes.

The role requires a relevant tertiary qualification and at least four years of experience in rehabilitation or counseling, with strong case management and EHR skills. Flexibility for evenings/weekends may be needed.

Competenze

• Relevant tertiary qualification in Psychology, Social Work, Counseling, or related field.
• Minimum 4 years of experience in substance abuse counseling or rehabilitation.
• Knowledge of addiction recovery models and relapse prevention.
• Strong case management, assessment and intervention skills.
• Ability to work in a multidisciplinary team and with families.

Mansioni

• Develop and manage individualized rehabilitation plans for clients.
• Coordinate delivery of therapeutic services including counseling.
• Liaise with clients, families and external agencies for continuity of care.
• Monitor progress and document milestones and challenges.
• Facilitate psychoeducational groups and life skills sessions.
