Job summary

Placements24 is seeking a proactive Community Care Coordinator in Vereeniging, Gauteng to assess client needs, develop care plans, and coordinate with service providers. The role emphasizes on-site responsibilities and local engagement within the community.

The ideal candidate will have relevant qualifications in Social Work, Nursing, or Community Health, with at least 3 years in care coordination, and strong communication and organizational skills.

Qualifications

• Must have a relevant qualification in Social Work, Nursing, Community Health, or related field.
• Minimum 3 years experience in care coordination, case management, or community support.
• Demonstrated knowledge of Vereeniging and Gauteng healthcare landscape.
• Excellent interpersonal, communication, and negotiation skills.
• Strong organizational and time-management abilities.
• Ability to work independently and in a multidisciplinary team.
• Proficiency in standard office software and electronic health record systems.
• Empathy, patience, and client advocacy commitment.
• Valid driver’s license and willingness to travel within the community.
• Ethical considerations in care provision.

Responsibilities

• Conduct comprehensive needs assessments for clients referred for care services.
• Develop individualized care plans with clients, families, and healthcare professionals.
• Coordinate and facilitate delivery of home-based care, therapy, and social activities.
• Liaise with doctors, nurses, social workers and other service providers for seamless coordination.
• Monitor client progress and review care plans to ensure appropriateness and effectiveness.
• Maintain accurate client records and documentation.
• Provide support and advocacy for clients and families.
• Organize and facilitate community support groups and educational sessions.
• Identify gaps in local service provision and contribute to new initiatives.
• Ensure compliance with regulations and organizational policies.
