Placements24 is seeking a dedicated Family Support Coordinator to empower families in Klerksdorp, North West. You will provide resources, guidance and advocacy, working with diverse families to build strengths and support networks.

Qualifications

• Qualification in Social Work, Community Development, Psychology or related field.
• Experience in family support, social services or community development.
• Knowledge of local resources and networks in Klerksdorp area.

Responsabilités

• Conduct needs assessments with families to identify challenges and strengths.
• Develop individualized family support plans in collaboration with clients.
• Connect families with community resources such as parenting classes and counseling.
• Provide ongoing support and advocacy during service engagement.
• Organize family group sessions and empowerment workshops.
• Maintain confidential client records and documentation.
• Liaise with schools, healthcare providers and agencies to coordinate support.
• Monitor progress and adjust support plans as needed.
• Promote early intervention and prevention within families.
