Job summary

Placements24 is seeking a compassionate Family Support Case Manager in Pretoria, blending fieldwork with office tasks in a Hybrid role. You will assess families, build outcome-focused service plans, and connect clients with housing, education, and healthcare services.

The ideal candidate has a social work or related background, strong empathy, and experience in trauma-informed care, with excellent communication and listening skills. Travel within Pretoria area is required.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
• 3+ years experience in case management, family support, or child welfare services.
• Experience conducting needs assessments and developing case plans.
• Understanding of child development, family dynamics, and trauma-informed care.
• Excellent interpersonal, communication, and active listening skills.
• Proficiency in case management software and Microsoft Office Suite.
• Ability to work independently and as part of a team.
• Valid driver's license and willingness to travel within Pretoria and surrounding areas.
• Commitment to ethical practice and confidentiality.
• Resilience and ability to manage emotionally demanding situations.

Responsibilities

• Conduct comprehensive assessments of family strengths, needs, and risks.
• Develop and implement individualized case plans in collaboration with families.
• Provide direct support, counseling, and advocacy for families.
• Connect families with essential community resources, such as housing, education, and healthcare services.
• Monitor family progress and adjust case plans as necessary.
• Maintain accurate and timely case documentation and progress reports.
• Collaborate with other professionals, including educators, healthcare providers, and legal representatives.
• Facilitate supervised family visits when required.
• Provide crisis intervention and support services as needed.
• Participate in team meetings, case consultations, and professional development activities.
