Job summary

Canadian Mental Health Association, P.E.I. is seeking a dedicated facilitator to support clients by sharing information on accommodation, safety, health, and medication policies, and by coordinating referrals.

You will help develop life skills such as budgeting, cooking, and medication management; assess progress with SPDAT; build partnerships with community and government agencies; and maintain accurate, confidential documentation for each person served.

Qualifications

• Adaptability and strong oral communication are required.
• Ability to work well with others in a team environment.
• Experience with case management, client services, and healthcare/human services.
• Proficiency with Microsoft Outlook and PowerPoint.

Responsibilities

• Inform clients about policies on accommodation, communication, safety, referrals, health, and medications.
• Develop life skills like cooking, cleaning, medication management, and budgeting.
• Connect clients with community resources based on individual needs.

Job description

Duties:

• Provides information to clients on policies and procedures related to accommodation, communication, safety, referrals, health, and medication.
• Development of life skills including but not limited to cooking, cleaning, medication management, budgeting etc.
• Connects clients with access to community resources based on individual needs.
• Assess and evaluates individual client’s progress using SPDAT system.
• Collaborates and develops partnerships with community organizations and government agencies for referral of clients as to available services
• Ensures proper documentation by keeping an accurate and up to date record of employment and educational supports provided to each individual served.
• Maintains confidentiality of information
