Placements24 seeks a compassionate and organized Community Care Coordinator to join our team in Richards Bay, KwaZulu-Natal. You will be the primary contact for clients, guiding them to essential social services, resources, and support networks within the Richards Bay community.

This on-site role requires hands-on community engagement, excellent interpersonal skills, and the ability to build trust with diverse clients. A background in social work or related fields is preferred.

Qualificações

• Qualification in Social Work or related field preferred.
• Proven experience in community-based or social support roles.
• Strong knowledge of KwaZulu-Natal resources and Richards Bay area.

Responsabilidades

• Conduct needs assessments for individuals and families requiring social support.
• Develop personalized care plans in collaboration with clients.
• Connect clients with community resources, including health services, education programs, and welfare support.
• Provide ongoing support and follow-up to ensure clients access services.
• Maintain confidential client records and case notes.
• Build relationships with local service providers and community organizations in Richards Bay.
• Organize and facilitate community outreach events and workshops.
