About the Role

Our client is looking for a highly skilled and dedicated Community Health Nurse Practitioner to join their innovative healthcare team, serving the Sandton, Gauteng area remotely. This role focuses on delivering accessible and high-quality primary healthcare services through telehealth and remote patient monitoring. You will be responsible for diagnosing and treating common illnesses, managing chronic conditions, providing health education, and coordinating care for patients in their homes or local clinics. The aim is to improve health outcomes and increase access to care for the Sandton community, particularly for those with mobility challenges or living in underserved areas. Key Responsibilities

• Conduct remote patient assessments via video consultations, phone calls, and secure messaging platforms.
• Diagnose and treat a wide range of acute and chronic health conditions.
• Prescribe medications and develop comprehensive treatment plans.
• Provide health counseling, education, and preventative care advice to patients and their families.
• Manage chronic disease programs, including diabetes, hypertension, and cardiovascular disease.
• Order and interpret diagnostic tests, such as lab work and imaging studies.

Requirements

• Master of Science in Nursing (MSN) with a specialization as a Nurse Practitioner.
