About the Role

Key Responsibilities

• Conduct health assessments, screenings, and diagnostic services for individuals and families.
• Provide immunizations, wound care, and management of chronic conditions.
• Develop and deliver health education programs on topics such as nutrition, hygiene, family planning, and disease prevention.
• Conduct home visits to assess patient needs and provide ongoing care and support.
• Collaborate with local clinics, hospitals, and social services to ensure continuity of care.
• Participate in community health surveys and data collection to identify health trends and needs.
• Advocate for patients' health needs and facilitate access to healthcare services.
• Promote healthy lifestyles and preventative health measures within the community.
• Maintain accurate patient records and document all services provided.

Requirements

• Registered Nurse (RN) qualification with current registration with the South African Nursing Council (SANC).
• Minimum of 4 years of experience in nursing, with at least 2 years in community health or public health nursing.
• Proven experience delivering healthcare services in diverse community settings in Western Cape.
• Strong clinical assessment and diagnostic skills.
• Excellent knowledge of public health principles, disease prevention, and health promotion strategies.
• Proficiency in managing common chronic diseases and infectious conditions.
• Exceptional communication, interpersonal, and teaching skills.
• Ability to work autonomously and as part of a multidisciplinary team.
• Valid driver's license and willingness to travel extensively within the Stellenbosch area and surrounding communities.
