Our client, a dedicated public health organisation, is seeking a compassionate and skilled Community Health Nurse to join their outreach team in Bloemfontein, Free State . Key Responsibilities

• Conduct health screenings, assessments, and basic medical examinations in community settings.
• Provide immunisations, administer medications, and manage minor wounds and ailments.
• Deliver health education on topics such as nutrition, hygiene, family planning, and chronic disease management.
• Identify health risks and provide interventions to prevent illness and promote healthy lifestyles.
• Monitor patients with chronic conditions, ensuring adherence to treatment plans and follow‑up care.
• Make referrals to clinics, hospitals, and other healthcare facilities as needed.
• Maintain accurate and confidential patient records in accordance with professional standards.
• Collaborate with community leaders, local clinics, and other health workers to coordinate care.
• Participate in health awareness campaigns and community engagement initiatives.
• Respond to community health emergencies as required.

Requirements

• Registered Nurse qualification and current registration with the South African Nursing Council (SANC).
• Minimum of 2-3 years of experience in general nursing, with a preference for community health or primary healthcare settings.
• Demonstrated experience in conducting health assessments, health education, and patient counselling.
• Strong understanding of public health principles and primary healthcare delivery models.
• Excellent communication, interpersonal, and active listening skills.
• Ability to work independently in community‑based environments and manage a varied caseload.
• Proficiency in record‑keeping and basic medical documentation.
• A valid driver's license and willingness to travel extensively within the Bloemfontein and surrounding Free State areas is essential.
• Certification in Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) is an advantage.
